Last Updated: [DATE]
1. Program Description
[COMPANY NAME] (“we,” “us,” or “our”) operates an SMS messaging program (the “Program”) that sends text messages to individuals who have provided their express written consent to receive such communications. Messages sent through this Program may include:
- Appointment reminders and confirmations
- Account notifications and alerts
- Service updates and status notifications
- [ADD ANY OTHER SPECIFIC MESSAGE TYPES YOU WILL SEND]
Promotional or marketing messages, if applicable, will only be sent to subscribers who have provided separate express written consent for such communications.
2. Consent to Receive SMS Messages
By enrolling in our SMS Program — whether through a web form, mobile application, text keyword, or other opt-in mechanism — you expressly consent to receive automated text messages from [COMPANY NAME] at the mobile phone number you provide.
Your consent is:
- Voluntary — not a condition of purchasing any product or service from us.
- Specific to the Program — limited to the message program(s) you enroll in.
- Specific to your number — tied to the phone number you provide at the time of enrollment. Consent for one number does not extend to any other number.
- Revocable — you may withdraw consent at any time. See Section 5 for opt-out methods.
If you enroll multiple phone numbers, your consent applies separately to each number. Consent for one number does not extend to any other number associated with your account.
3. Message Frequency and Costs
Message frequency: You may receive up to [NUMBER] messages per month, depending on your account activity and the program(s) you are enrolled in. Message frequency may vary.
Message and data rates: Standard message and data rates may apply to all messages sent and received. These charges are billed by your mobile carrier and are your responsibility. [COMPANY NAME] does not charge a separate fee for SMS messages.
Supported carriers: Our SMS Program is available on most major carriers in Canada and the United States, including but not limited to Rogers, Bell, Telus, Fido, Virgin Plus, Koodo, AT&T, Verizon, T-Mobile, and US Cellular. Carrier support may vary and is subject to change.
International messaging: Standard SMS messaging is available to Canadian and US mobile numbers. International messaging outside of Canada and the United States is not currently supported through this Program.
4. How to Enroll (Opt In)
You may enroll in our SMS Program through the following methods:
- Web form: Complete the SMS enrollment form on our website at [URL] and check the box confirming your consent to receive SMS messages.
- Text keyword: Text [KEYWORD] to [YOUR PHONE NUMBER].
- [OTHER METHOD]: [DESCRIBE]
Double opt-in confirmation: Upon enrollment, you will receive a confirmation text message asking you to reply YES, CONFIRM, or START to verify your consent. Your enrollment will not be activated until we receive your confirmation reply. This confirmation request will expire after [24/48] hours if not confirmed.
You must be the account holder or have authorization from the account holder to enroll a mobile number in our Program. By enrolling, you represent that you are 18 years of age or older, or have obtained parental or guardian consent if applicable.
5. How to Opt Out (Unsubscribe)
You may revoke your consent and opt out of receiving SMS messages from us at any time using any of the following methods:
- Text STOP: Reply STOP to [YOUR PHONE NUMBER] from the enrolled mobile number.
- Other keywords: You may also text QUIT, CANCEL, UNSUBSCRIBE, or END to [YOUR PHONE NUMBER].

We will process your opt-out request within 10 business days of receipt. Following your opt-out, you will receive a single confirmation message acknowledging your request. No further messages will be sent after that confirmation, except as required by law.
Opting out of SMS messages does not affect any other communications you may receive from us (such as email or phone calls), nor does it affect your account status or any products or services you have purchased.
If you have enrolled multiple phone numbers, you must opt out each number separately, or contact us to request a global opt-out from all numbers associated with your account.
